Move-In Instructions for Memory Care Residents

New residents will need to obtain a medical report with a chest X-Ray (current within 90 days) and a list of current medications, to be completed and signed by your physician. The Hospitality Office can provide blank forms for your doctor. If the senior is at the hospital, medical records may be sent via FAX at (248) 656-8160.
